Forms and Applications
Reservations can be made online, over the phone, in person at one of our Permit Office locations, or via mail or fax.
Completed application forms may be submitted in one of five ways:
- In person at either of our Permit Office locations during operating hours;
- Online at www.ParkPermits.org;
- By calling the Silver Spring Permit Office at 301-495-2525;
- By mailing your completed application form to Montgomery Parks, M-NCPPC, Park Permit Office, 9500 Brunett Avenue, Silver Spring, MD 20901; or,
- By faxing your completed application form to 301-495-2484.
NOTE: All ballfield applications must be submitted to the Silver Spring Office.

Athletic Fields
- Information on Athletic Field Permits and Fees: The Planning Board approved the new hourly fee schedule proposed by staff at their Thursday, October 23, meeting. These new fees will go into effect on January 1, 2009, for the season starting April 1, 2009. View the staff response memo submitted to the Board. A full copy of the original Board Packet presented at the September 15 meeting, including memo and attachments can also be downloaded for review.

NOTE: We recommend that you visit the park activity building in advance of making a reservation to ensure the building fits your needs. Keys are available at our Permit Office locations and can be picked up between the hours of 8:30am - 3:00pm daily. Photo ID and a key deposit of $25 will be required. The key must be returned to the same location by 4:30pm the same day.
